QUALIFICATIONS:

 

Minimum of a Bachelors Degree, in the Human Service field.  Prefer two years experience in the care and treatment of the Seriously Mentally Ill Adults or SU Adults; ability to express self clearly and effectively in oral and written communication; ability to work independently and the ability to function within a team.  Must be 23 years of age and have a valid Alabama Driving License and a clean driving record.

 

DUTIES AND RESPONSIBILITIES:

Build collaboration with other providers/service agencies/services in the community to assure client’s needs are met.

Establish relationships with community agencies and organizations; provide awareness of programs and services.  Assist clients in obtaining the services within the community.

Provide Case Management services to adults within the SU Program.

Schedule home/office visits on at least a monthly basis.

Follow up with No Show clients.

Obtain relevant information from clients and their families in order to develop service plans for clients.

Prepare appropriate service documentation for Clinical Record.

Documentation of events; preparation of progress notes, consults, etc.
